Located near Cranberry Township and the intersection
of the Pennsylvania Turnpike and Interstate 79, The Worthington
at Adams is an assisted living facility specializing in care
for seniors with Alzheimer’s or other memory impairments.
The Worthington’s Memory Care program is designed to meet
the unique needs of seniors with Alzheimer's disease or other
memory impairments. For remembering the past and multi-sensory
experiences, we create everyday routines that are comfortable
and pleasant.
MEMORY PROGRAM HIGHLIGHTS

PERSONALIZED SERVICE AND ASSISTANCE
We understand that each resident is an individual with unique
needs and preferences. That's why we meet with residents
and families and conduct an assessment – prior to move-in
and at regular intervals after move-in, to understand the need
for assistance and support in the following areas:
- Personal hygiene and grooming
- Showering or bathing
- Dressing and clothing selection
- Socialization and activities
- Medication administration
- Mobility and transferring
- Continence management
- Monitoring of safety
- Assistance with eating
This assessment helps determine the appropriate care and service
that preserves each resident’s dignity, encourages independence
and best enables freedom of choice. The assessment results
in determining a specific service-level based on the amount of
assistance provided.
Upon completion of the assessment, a team comprised of the Executive
Director, or Resident Care Coordinator, Health-Care Coordinator,
and our designated care Manager will prepare an individualized
service plan that is monitored on a daily basis.
Amenities and Services
Continence Management
Working together with the resident as well
as their family and physicians, we will determine an appropriate
care plan. We
will continually assess this plan and provide suggestions for
products and management should a seniors needs change.
Medication Management
We order, manage and administer medication,
depending on the needs of each resident, using approved pharmacies.
Respite Care
For those seniors that need assisted living services
on a short-term basis, such as while family is on vacation or
the resident is recovering from an illness or hospital stay
